This is to be completed by a Grace Community Church leader/staff member or close friend (non-family member) who works with or knows the applicant well.

Note: The purpose of this team is to train and further develop men and women who are doing well in the ministry in which they are currently involved. They need to be developing as multipliers; whether they are already in leadership roles or being trained for leadership. If they are new in the ministry, but exceptionally eager and teachable they may also qualify. Your honest and straightforward answers will help us design a team which will best develop these men and women and most effectively do God’s ministry. Be assured your answers will be held in strict confidence.
